How to become a Member of Philhealth? – Requirements and Steps to Register

Do you wanna be a member of Philhealth and want you and your beneficiaries avail of the benefits? Know the steps and requirements here either if you are currently unemployed, employed, or an OFW.

Philhealth is the leading health insurance company in the Philippines and it is administered by the government so many Filipinos rely on it so much. The monthly contribution is so affordable and it is the cheapest in the country so being an active member of Philhealth help so much.

Philhealth Requirements for Unemployed Members (Individually Paying Member)

If you are currently unemployed or either self-employed, you can register as an Individually Paying Member. This is also being referred as “voluntary member” by some. To become one, you must submit the following requirements at the Philhealth office near you. If you are a foreigner living in the Philippines, you can also become a member classified under this type.

  • PMRF (Philhealth Member Registration Form) – Accomplished and properly filled out. This form can also be downloaded online for free
  • Photocopy of NSO authenticated Birth Certificate or valid IDs or any supporting documents to prove identity
  • Photocopy of supporting documents for your beneficiaries
  • Photocopy of Alien Certificate of Registration (ACR) issued by the Bureau of Immigration (for foreigners only)

You will then be given and issued your Philhealth Number and will be asked to pay your contributions. Keep your PN because this will be used for life. Make sure you will be paying your premiums continuously to ensure you can avail benefits any time.

Requirements for Employed Members

The requirements for employed members are the same as listed above for Individually Paying Member. Just include your employed status in the PMRF.

Philhealth Requirements for OFW

For OFW (Overseas Filipino Workers), you can register and pay your contributions once you register at the POEA as OFW. Requirements are:

  • PMRF
  • Supporting documents of beneficiaries
  • Any proof of being an active OFW

If you already are a member of Philhealth, you can just pay your contributions and update your status as OFW.

If you are outside the Philippines and wants to become a member of Philhealth, you can just submit the following requirements to any Philhalth office:

  • PMRF
  • Philhealth Premium Payment Slip (PPPS)
  • Photocopy of acceptable valid IDs
  • Pay your contributions to accredited collecting agents of Philhealth abroad

Sponsored Members of Philhealth

Philhealth Members Sponsored by LGUs, National Government, congress and Indigents belonging to the lowest 25% of the Philippine population can become Sponsored Members by submitting the usual requirements to their sponsors or at Philhealth office

    Yung husband ko po has been working bpo going 5 years. Ngayong required mag submit ng mdr sa philheatlh ang mdr.po namin is indigent registered na hindi tinatanggap ng employer. Pumunta kami ng Main office ng philhealth para mag request ng formal economy mdr pero hindi daw sila makaka pag bigay. Paano ho kumuha ng formal economy mdr?

    Ask ko lng po,Dito po ksi ako abroad nakatira Unemployed po ako now, yung 2 kids ko nasa pinas (minor) Sakitin sila. Gusto ko sana sila kuhanan ng PhilHealth. Qualify po ba ako makajoin sa philhealth pra sa mga anak ko khit wala ako sa pinas at sa abroad nakatira.?

  7. Hi, nung nag apply po ako ng Philhealth naka categorized ako as individually payee. Then 2 months after nag karoon ako ng job. upon checking my account online, naka individually payee parin ako then blanko yung employer ko pero may hulog naman yung contribution ko. Ang tanong ko po, ok lang po ba yun? hindi po ba ako mag kakaron ng issue if one day mag stop na ko mag work then hindi ko na mahuhulugan contribution ko?

Goodmorning ma’am Fehl, can i ask something, employed na po ako for almost 2 years,, is it possible po ba na i can have 2 PHIlhealth, kasi i visited once yung office ng philhealth dito samin to request for philhealth i.d pero they just issue me MDR kasi nakabilang daw po ako sa indigency people, pero nagkakaroon naman po ako ng contribution kasi dinededuct na po sa salary yung contribution.

    • Hi. Birth certificate mo and yung Birth certificate ng qualified dependents mo (kids) and your valid ID. The latest premium is 200/month. You can pay in advance for quarterly or annually

      • Hi po, my mother has parkinson and currently in the hospital because of a minor operation, hindi po sya member ng Philhealth, she is considered PWD po diba, can we get help from Philhealth po to settle our bills and paano po kaya gagawin ko to get the help. Thank u po

        • Hi, if she’s a senior citizen and a beneficiary of any of her children under their Philhealth, then may makukuha sya. I suggest if she’s a senior citizen, let her register sa Philhealth. And if PWD, let her register sa DSWD as PWD para may discounts sa medicines and stuff. God bless!

    • Vacationing and re-hired overseas Filipino workers will have to pay their premium contributions to the Philippine Health Insurance Corporation (PhilHealth) separately from their payments to the Philippine Overseas Employment Administration (POEA) for their Overseas Employment Certificates (OECs).This development is in line with the decision of the POEA to implement a single collection policy starting on the same date. The POEA will only be collecting fees for OEC and no longer for PhilHealth and Pag-IBIG Fund.
      OFW-members shoul pay their PhilHealth premium contributions first before going to the POEA for their OECs. This will secure their social health insurance coverage, as well as that of their legal dependents, for their peace of mind.
      Entitlement to health care benefits for OFWs and their legal dependents begins on the day the premium is paid, and is effective for the period covered by the contribution.

    Requirements po kasi yun sa pinapasukan ko, but I’m not yet hired.

    actually, nagregister po ako online, i checked private, and then they’re asking me if I do have PEN, then EMPLOYERS NAME. I’m not even hired yet!

    My point, , is there any online Philhealth registration for new members na hindi pa hired ng company? Required po kasi ng company ko. Please reply.

    • you do not log in first. you can select the register button and then filled up the form and follow the instructions given. if you are finish then select the submit button tapos may malabas pa yan na mga something fifilled upon mo and then makikita mo na ang password mo .

    • Yung importante lang naman ay ang Phileahlth number and active member ka po. You don’t need Philhealth ID, just master your Number

    • Request to change of surname or married status. Go to Philealth and bring supporting docs like NSO Marriage CErtificate and ID bearing your new surname

I just registered yesterday to get my Philhealth Member Data Record but the officer just gave me a piece of paper that is not the Member Data Record as far as I know (it doesn’t have the list of heirs and it’s just like a letter with my Philhealth number) and the membership status was Informal Economy Informal Sector even though I checked Formal Economy Private. (I just checked it when I got home cause I’m so tired when I register at Philhealth, my mistake I guess for not checking) What should I do with it cause our company is requesting for my Philhealth Member Data Record/ID? Thanks. (first job)

    • Give that paper to your employer, they just need your Number anyway for them to register you as one of their employees. Also, keep a copy of your Philhealth number because you’ll be using it all your life or when you change employer in the furture

    I already accomplished my Philhealth by visiting one of their branches, I filled up the form, waited for few minutes, and I received a letter with my lifetime PIN (Philhealth Identification Number) upon accomplishment..

    however, my soon-to-be company is requiring me to accomplish a “Signed Philhealth Member Registration Form” , “Two photocopies of Philhealth ID or MDR” but I don’t have any idea how to get that ID or MDR, note that I was already issued a PIN from one of the branches.

    Do I need to register online and update my status? Thanks in advance! Any help would be appreciated.

    • same problem here. I am a newly hired employee and my agency wants me to submit MDR. Can i have MDR even if i didnt have any payments yet since i am a new member. i have my PIN already secured from online registration. enough na po ba yung PIN lng isubmit sa agency or MDR tlga kaso nga po new member pa lng po ako. Galing na po ako philhealth office sbi e mgbayad daw ako 600 for me to have an MDR. Ako po ba dpat gumawa nun or yung agency na. please enlighten me about this since the agency is expecting me to submit it on monday. Your response will be highly appreciated po. Thanks in advance

      • Since you are not yet active member, you won’t be able to have premiums recorded on your account thus no MDR. Tell your agency that you are only about to start paying premiums through your employment with them.

    Just want to ask po.

    I am already a philhealth member since 2010. Now, I registered online thinking that it was like SSS that i have to create an online account to view my contributions or update my details etc. But I was surprised when I received their email advising me to wait for their reply to know my PIN. And wala pa din palang application ang Philhealth for viewing contributions. So, I immediately sent an email to the address they provided as contact and i inquire and told them to Cancel my online registration to avoid me having 2 different PINs. I think the law prohibits having multiple PINs?

    sa ngaun wala pa pong reply from philhealth. But i am hoping na makita nila ung records ko at hindi na i allow ung online registration that i made. I dont want to encounter conflicts in the future. Hay. It is so stressful.. XD i should have researched if philhealth already has that application before goiing on their website. lesson learned. Hope you can share what you know about this instance. Im worried. :/

    • I suggest you go to Philhealth office and verify if the registration you did went through. If it did, request to cancel it and just use your old PIN

    • I suggest you update your account before you pay para updated and pay at Philhealth mismo para Official ang receipt. Pag bayad center kasi nabubura ang receipt

Dati po ako member ng philhealth pero na stop na almost 2 year kasi I found out na iba pala apelyido ko sa NSO at yun ang totoo kaso lumaki ako na iba ang gamit na apelyido at un ginamit ko sa lahat ng ID’s including this philhealth…ano maigi at mabuting gawin? Hi, matatanong po ako,dati akong government employee,nag early retirement ako to work abroad,2001,dati nagbabayad ako ng philhealth,last payment ko ay 2004.Maaari po bang ireactivate ang membership ko with the same beneficiaries?Ano po ang mga kailangan ko or requirements to reactivate my membership just in case?Sana po maaasahah ko ang inyong sagot sa lalong madaling panahon.

    • If you don’t have a Philhealth number yet and ever, you must register first to become a member of Philhealth to have a number. PMRF is the form you need

    • I suggest you go to Philhelath and update your account as senior citizens are now automatically covered to Philhealth benefits. If you won’t update your account, Philhealth won’t know you’re a senior.

I even used the Medicare benefits in 1995 when I gave birth to my eldest. Now that I am 49 years and self-employed, member of SSS.     • There is now a new law granting automatic Philhelath membership to all senior citizens. Your mom is 60, she will be automatically covered. You must help her go to Philhelath and update her membership for the new law for senior citizens. Cheers!

      • Hi ask ko lang member po yung parent ko sa phil health bali indigence sila. Paano kung expired na ang phil health card pwede parin bah yun magamit. Wala kasing pakialam dswd sa amin. Anu dapat gawin namin. Pag over 18 na di na po bah pwede isama sa phil health ng parents

        • Philhealth card is just a card. What matter is your parent is paying her Philhealth monthly premiums. If she is, then she can always avail benefits. For 18 and above, you can already register for your own Philhealth account, if you’re employed, submit your Philhealth number to your employer, if not employed, register as Individual Paying member (voluntary)

  87. Hello, question lang po regarding requirements for registration for self-employed member. Nakalagay kasi dun sa nabasa ko na need ng NSO or Valid ID. Does this mean ok lang kung ID lang ang dalhin ko, kahit wala nang NSO? If so, How many Valid IDs do I need to bring?

    • Yes, NSO BC and valid ID are the basic requirements for first-time applicants. Bring 2 valid IDs para if ever di pwede yung 1, you still have another one to show them

    My dad used to be a member but his contributions stopped for four years now.
    Now he’s confined and hindi magamit yung PhilHealth nya coz di na xa active.
    If in case i.update nya again ang membership nya, is it possible na pwede nya mapa.reimburse ang nabayaran nya sa pagka confine nya ngayon?
    And if yes, mga ilang months po bago makuha nya ang reimbursement?

    • If the member is not active, he/she cannot is not eligible for benefits. Philhealth is an insurance company, if you are not insured (active), you cannot get benefits

    • If you’re not minor anymore yes, you can register as Individual Paying member. Use your supporting documents (Postal ID, NBI clearance, etc.), I think they accept them for anyone who doesn’t have IDs

    • BIR ID is not an acceptable ID. Submit SSS, GSIS, Driver’s license, passport or any other government ID. They require NSO authenticated but you can try

    • You can still pay the month of May this June. It’s not yet considered late. If you want to switch into Individual Paying Member, go to Philhealtha nd file PMRF and update your new status as Individual Paying member or voluntary member

    • Yes, it is advisable you resume it to become active and to be eligible to benefits. Just fill out PMRF to update your new membership status as self-employed or individual paying member if you are voluntary member. I know right now the minimum contribution is P150 per month

Hi just wanna ask. what if member napo nang Philhealth tapos na stop na ang pag pay premium contribution for 6 years na but planning to pay 12 months of premium contribution this month, can I avail the benefits if in case need magpa-hospitalized this month? (huwag naman po sana hehe) Thanks!

    • Just bring the mentioned requirements in the article and fill up a form called PMRF to register as Individual Paying Member, then start paying your contributions. If you’re working, just update your membership into EMPLOYED

    • For retirees or old age who did not meet the required 120 months for Lifetime Members, you can apply as Individually Paying Member instead. You can also apply as Individually Paying Member if you are not a dependent of a Philhealth member. In your case, pwede po kayo as IPM

  127. yung husband ko member na ng philhealt since 1999… nagamit din nmin yung philhealt niya nung ma admit sa hospital anak namin… pero nung 2008 nagresign na xa sa work nia hindi na namin yun nahulugan… pwedi pa ba naming ituloy yun dati niang number or nid niang mag apply ulit ng new..??

    • Yup it is important and advisable na ituloy kasi you need to be an active member to be eligible for Philhealth benefits. Tulad din ng SSS yan. Pag di ka active, di ka eligible sa benefits. You dont need to apply for new Philhealth number. Just use your old Philhealth number and update your membership by filling out PMRF form. Tick the box “for updating” purpose. You can download the form here: http://www.philhealth.gov.ph/downloads/membership/pmrf_revised.pdf and submit to Philhealth. Bring your valid IDs

    • According to Philhealth, retirees or 60 and above, you may be qualified to be a Lifetime Member if you are:

      1. Retirees/ Pensioners from the Government Sector

      Old-age retirees and pensioners of the GSIS, including non-uniformed personnel of the AFP, PNP, BJMP and BFP who have reached the compulsory age of retirement before June 24, 1997, and retirees under Presidential Decree 408.
      GSIS Disability Pensioners prior to March 4, 1995.
      GSIS Retirees who have reached the age of retirement on or after March 4, 1995 and have at least 120 months PhilHealth premium contributions.
      Retirees and Pensioners who are members of the Judiciary who have reached the age of retirement and have at least 120 months PhilHealth contributions.
      Retirees who are members of Constitutional Commissions and other Constitutional Offices who have reached the age of retirement and have at least 120 months PhilHealth contributions.

      2. Retirees/ Pensioners from the Private Sector

      SSS Pensioners prior to March 4, 1995.
      SSS Permanent Total Disability Pensioners prior to March 4, 1995.
      SSS Death/ Survivorship Pensioners prior to March 4, 1995.
      SSS Old-age Retirees who have reached the age of retirement on or after March 4, 1995 and have at least 120 months PhilHealth premium contributions.

      3. Uniformed Members of the AFP, PNP, BJMP and BFP

      a. Uniformed personnel of the AFP, PNP, BJMP and BFP who have reached the compulsory age of retirement before June 24, 1997, and retirees under Presidential Decree 408.
      b. Uniformed members of the AFP, PNP, BFP and BJMP who have reached the compulsory age of retirement on or after June 24, 1997, being the effectivity date of RA 8291 which excluded them in the compulsory membership of the GSIS and have at least 120 months PhilHealth premium contributions.

      4. Former employees of the government and/ or private sectors who have accumulated/ paid at least 120 monthly premium contributions as provided for by law but separated from employment before reaching the age of 60 years old and thereafter have reached 60 years old.

      5. Former employees of the government and/ or private sectors who were separated from employment without completing 120 monthly premium contributions but continued to pay their premiums as Individually paying Members and completion of the required 120 monthly premium contributions and have reached 60 years old as provided by the law.

      6. Individually paying Members, including SSS self-employed and voluntary members, who continued paying premiums to PhilHealth, have reached 60 years old and have met the required 120 monthly premiums as provided for by law.

      7. Retired underground mine workers who have reached the age of retirement as provided for by law and have met the required premium of at least 120 months contributions.

      8. PhilHealth members of Overseas Workers Program and Sponsored Program who have completed the required premium of at least 120 months contributions and have reached the age of retirement as provided for by law.

      • I retired at age 57. Therefore, I have to continue contribution as individually paying member. How can I obtain my former Philhealth Member. My former employer Career Shipmanagement Philippines, do not have records about my philhealth number. Thank you

        I was former OFW who stopped working since January 2014.

  129. nagregister po ako last january 27,2014 morning sa online registration ng Philhealth. Wala po akong natanggap na confirmation message. hindi ko po tuloy alam ung Philhealth number o naregister po ba talaga ako. pero ang sabi pagkatapos ko magregister may nakalagay sa website page na “MEMBER INFORMATION SUCCESSFULLY SAVED”. ANO PO BA ANG DAPAT KUNG MABISANG GAWIN?

    i just want to ask, kakamember ko lang po kahapon and i was shocked of the super fast process…i brought my nso birth cert and id pa naman because i thought it will be needed. but i just filled out a piece of paper my just my name, birthdate & place, address, and civil status…i just gave it to a personnel, she input the data in the computer, printed out a paper that wrote i’m now a member and has a PN already…it only took 2mins i guess…
    so my question is, how will i process my parents as my dependants and i was planning to pay already for this whole year…can you give me an insight about this…i deeply appreciate your time and effort reading this ang hoping for a reply…
